Insurance Questions

Dr. Kosman is an out-of-network provider for all insurance providers and therefore does not work with or bill insurance companies directly. You are responsible for paying the full amount of the fees at the time of service.

Cancellation Policy

Your appointment time is reserved for you. Missed appointments or cancellations made less than 24 hours in advance are billed at the standard rate. Otherwise, we are happy to help you reschedule for the best fit in your schedule.
